Media Library Reservation Form

This form can be used to book 1 or 2 titles for a single date. Only faculty members and active TAs (with a current letter from the department verifying the course they are teaching) may book films.

Submitting this form does not guarantee the booking. The request will be checked by the librarian to ascertain whether the item is available as desired. Expect a reply in your own e-mail.
