Scan Document Development
Several options exist to prepare and use customized scan documents.
Forms printed by NCS Pearson can be scanned by the NCS scanners located in Turlington Hall. The cost of scanning is 2 cents per sheet plus a one-time programming fee of approximately $100 (depending upon the length and complexity of the survey forms). Departments receive a raw data file as output.
Academic Technology staff can help design customized survey forms. Once designed, faculty members will be directed to a contact at NCS Pearson, by the Academic Technolgy staff, in order to complete the form printing process and purchasing. The NCS Pearson service cost varies depending upon the length and complexity of the survey and the quantity printed. Because these forms are NCS Pearson froms, they can be scanned by Academic Technology Testing and Evaluation staff. An additional cost will be charged for Academic Technolgy to program the application for the scanners, and print and scan the forms. Departments receive a raw data file as output.
Pre-printed survey scan forms can be sold to a Department. The Department can then print their survey questions onto the form. The forms are sold in 1120 Turlington Hall for a cost of $33 per 500 forms. The price includes the cost of scanning the forms. Departments can receive a raw data file and/or an item count as output.
Generic answer documents can be used in combination with a separate survey form prepared by the Department. These forms are sold in 1120 Turlington Hall for a cost of $15 per 500 forms.
